How it works
Support begins with a relaxed conversation to understand your situation, priorities, and any concerns. We then carry out a full consultation and assessment, allowing us to create a personalised support plan that can start gently and adapt over time. Support is delivered flexibly, at home or in the community, one-to-one or in small groups, and is reviewed regularly as needs change.
